The Montgomery County Veterans Service Commission transportation service is for Veterans with verified medical appointments with any medical provider in Montgomery County who do not have access to other means of transportation for their appointments.

Once the following required information is received, the application (download here) will be processed for verification. Please allow up to 7 business days to process a first-time application.

  • The client must provide a copy of all DD Forms 214. The client must have an Honorable or Under Honorable (General) character of service, which must be visible on DD Form 214. If the client has been previously assisted by MCVSC, the client will need to verify that a copy is on file with MCVSC.
  • Proof of residency information must be provided. That can include a utility bill, VA letter, or bank statement with the Veteran’s current address showing residency in Montgomery County for at least 90 days.
  • The client must provide a valid state photo ID.
  • The client must provide a copy of the scheduled medical appointment with the location, date, and time of the appointment. MCVSC is not responsible for contacting and obtaining medical information.

MCVSC does not provide emergency medical transportation or transportation to appointments that require a signature release to discharge the client from medical care.

Once the application is submitted to our office, it will be verified by management for approval. A member of the transportation team will contact the applicant to coordinate travel.
